Job Summary- The Transitional Care & Dietary Scheduling Assistant / Medical Assistant is responsible for coordinating Transitional Care Management (TCM) services and dietary-related appointments for our clinics. This position supports continuity of care by ensuring timely patient follow-up after hospital discharge and assisting with administrative and clinical functions that enhance patient outcomes, provider efficiency, and patient satisfaction.

Primary Job Responsibilities

  • Coordinate and schedule Transitional Care Management (TCM) follow-up appointments for recently discharged patients within required timeframes.
  • Conduct post-discharge outreach calls to patients to assess needs, confirm follow-up appointments, and reinforce discharge instructions.
  • Schedule dietary and nutrition-related appointments for clinic patients as directed by providers.
  • Maintain accurate and timely documentation within the electronic medical record (EMR) system.
  • Communicate effectively with providers, nursing staff, case management, dietary services, patients, and family members to ensure continuity of care.
  • Verify patient demographics, insurance information, and obtain required authorizations when necessary.
  • Assist providers and nursing staff with patient intake, vital signs, medication reconciliation, and other clinical support duties as needed.
  • Monitor provider schedules and assist with coordinating patient care needs to improve clinic workflow and efficiency.

Minimal Qualifications:

  • Required: High School Diploma or equivalent. 
  • Required: 1+ years of experience in a health care environment
  • RequiredBLS 
  • Preferred: Certificate Medical Assistant or Nursing Assistant Program
